PC is 74-year-old woman who fell and broke her femur.  Discharged to rehab facility where she fell again causing second break.  Hospital did not note that the prescribed blood thinners cause light-headedness and dizziness which led to the fall.  Upon return to ER, the hospital discover multiple blood clots in her lungs which (presumably) were missed the first time.
